"Private Dentistry" magazine (UK) will feature our dental patient form (which, incidentally, was developed on this board) in their October edition, thanks to Andrew of www.fearfreedentistry.co.uk fame!

Just wondering whether it will feature just the short version of the form or also the items that didn't make it onto the form?
